Function tickets incurring a fee will require payment via credit card (Mastercard, Visa or American Express) on the completion of this form and a 1.5% surcharge will apply. All fees are quoted in Australian Dollars (AUD) and are inclusive of GST.

Table bookings include:
• 10 dinner tickets
• Your organisation's name displayed on the table alongside the dinner sponsor logo
• Table located towards the front of the room

Booking contact

The booking contact will be responsible for collecting the tickets (10) from the registration desk and distributing to your guests prior to the dinner. Guests must present tickets on entry to the dinner.
